- ADHD assessment form/ standardized questionnaire (Adult or Pediatric)
- Test of Variables of Attention (In person)
- Gathering of additional information from family member or school teacher (if indicated)
- Review of prior psychiatric record
- Feedback session
- Evaluation report
- Cost= $500 (Insurance not accepted)
Note: Patients will receive a superbill which can be submitted to insurance for reimbursement (may or may not be reimbursed by insurance).
